With a few easy steps, you can easily locate the right SGS service for your client based on:

• The type of specialized service required (Geriatric Psychiatry Outpatient Services, Case Management, Geriatric Outreach Team, Inpatient Geriatric Rehabilitation Unit, Elder Abuse, etc.)
• The geographic area in which a client lives based on street address
• Type of assessment and/or treatment available (Functional, Neurological, Pain, Psychiatric, Memory, Medication Management, etc.)
• Type of professional expertise on the team
• The eligibility/exclusion criteria for the service
